Резюме
Совместное принятие решений (Shared decision making – SDM) в психиатрической помощи включает совместную работу врача и пациента над принятием решений. Ключевые элементы SDM определены, средства помощи принятия решений разработаны, и SDM рекомендовано в психиатрической практике на политическом уровне. Однако, реализация его до сих пор остается ограниченной. Обычно в пользу SDM приводятся два обоснования. Клиническое говорит о том, что SDM приводит к улучшению результатов, но доступная эмпирическая доказательная база пока неубедительна. Согласно этическому, SDM является одним их прав пациентов, но клиницисты должны балансировать между этическими приниципами автономии и справедливости и пользой и не причинением вреда. Считается, что SDM – поливалентная социологическая концепция, которая описывает идею управления поверхностным, но не глубоким соглашением между заинтересованными сторонами. Внедрение SDM в психиатрическую практику является как культурной, так и технической проблемой. Выделены три направления: обеспечение широкого доступа к качественным средствам помощи принятия решений; интеграция SDM с другими вмешательствами, способствующими восстановлению; реагирование на культурные изменения, так как у пациентов развиваются ожидания нормальной жизни. Определены два подхода, которые могут информировать о реакции в психиатрической помощи на культурные изменения – социальный маркетинг и индустрия гостеприимства.
