Аннотация / Abstract
Резюме. Статья посвящена моносимптоматическим ипохондрическим психозам (МИП) – редким формам психических расстройств, реализующимся дерматозойным (зоопатический) бредом и дисморфическими расстройствами, распространенность которых оценивается в 0,08–0,09% и 0,7–2,4% соответственно. Формируясь по механизмам развития паранойяльного бреда вне связи с патологией внутренних органов или кожных покровов, МИП определяются убежденностью в соматической природе страдания в сочетании с бредовыми нарушениями рецепции, аномальными телесными сенсациями, телесными и обонятельными галлюцинациями и расстройствами схемы тела и сопровождаются аутодеструктивным поведением. МИП характеризует монотематичность бредовых построений, простота и однотипность телесных сенсаций, хроническое течение при отсутствии тенденций к генерализации и формированию негативных изменений. В контексте нозологической принадлежности МИП может квалифицироваться в рамках самостоятельного паранойяльного расстройства или шизофрении. Приоритетная роль в лечении МИП отводится психофармакотерапии. Выбор психотропных средств и интенсивность фармакологического воздействия зависят от типологической и нозологической принадлежности МИП. В случаях дерматозойного бреда наиболее эффективны антипсихотики. При дисморфоманических расстройствах наилучший результат лечения обеспечивается серотонинергическими антидепрессантами как в режиме монотерапии, так и в комбинации с антипсихотиками.
